This secure portal is provided for authorized healthcare facilities, employers, and credential verification representatives seeking to confirm an individual's completion of training and certification through our VA-BC® VASCULAR ACCESS BECOME CERTIFIED® program.

NOTICE TO HEALTHCARE FACILITIES, EMPLOYERS, AND AUTHORIZED REPRESENTATIVES

Phoenix Vascular Access, LLC provides this secure verification portal for authorized healthcare facilities, employers, and credentialing representatives seeking confirmation of an individual's VA-BC® training and certificate completion records. We are also under strict contract rules with hospital and healthcare facilities and must obtain authorization directly with providers to release information.

CERTIFICATE HOLDER AUTHORIZATION REQUIRED

All requests for nonpublic training, certification, or certificate completion information require authorization from the certificate holder before information is released.

The requesting organization must obtain the certificate holder's written permission for the verification request, including when the request is associated with a pre-employment background check, professional credentialing, or continued employment evaluation.

The requesting organization is responsible for compliance with applicable federal and state laws, including the Fair Credit Reporting Act (FCRA), where applicable.

VERIFICATION AND APPROVAL PROCESS

Upon receiving a verification request, Phoenix Vascular Access, LLC will review the submitted information and contact the certificate holder through the contact information maintained in its records to request confirmation and authorization.

Verification information will be released only after the request has been authenticated and the necessary certificate holder authorization has been obtained.

Phoenix Vascular Access, LLC and VA-BC® Training Center, LLC reserve the right to deny, delay, or request additional documentation for any verification request that does not satisfy their verification and authorization requirements.

CONFIDENTIALITY AND AUTHORIZED USE

Any verification information released is intended solely for the authorized purpose identified in the request. Unauthorized disclosure, distribution, reproduction, or use of certificate holder information is prohibited.
